This service provides an online medical assessment for a medical certificate request. It does not replace your regular GP or an in-person examination. A registered Australian doctor will review your answers and decide whether a certificate is appropriate.

What this certificate can be used for

Medical certificates are generally used to support time away from work, school, university, or carer’s leave due to a short-term illness or medical issue, based on the information you provide.

Important - certificates are not always issued

A medical certificate may not be issued if:

Your symptoms suggest you need urgent care or an in-person assessment
Your information is incomplete, inconsistent, or unclear
The doctor cannot clinically justify a certificate from the details provided
You request a certificate that is unreasonable (e.g., excessive time off without clinical basis)

Your responsibilities (please read carefully)

By proceeding, you confirm that:

The information you provide is true, accurate, and complete to the best of your knowledge
You are requesting a certificate for a genuine health-related reason
You understand a doctor may request more information or recommend a consultation before issuing anything
You will provide correct details about who the certificate is for (yourself or someone else)
You agree to seek medical care if your symptoms get worse or do not improve.

Limitations of online assessments

You understand that:

The doctor is relying on your answers, without a physical examination
The certificate may be based on reported symptoms rather than confirmed diagnosis
The doctor may recommend an in-person review with a GP/urgent care if appropriate

Safety first - urgent symptoms

If you have severe or worsening symptoms, you must seek urgent medical care immediately:

Call 000 in Australia for emergencies, or attend the nearest Emergency Department.
This includes (but isn’t limited to) chest pain, difficulty breathing, signs of stroke, severe weakness, confusion, severe dehydration, or sudden vision changes.

Backdating and dates

Certificates are usually issued for the assessment date and/or a clinically reasonable period based on your symptoms.
Backdating may be limited or declined at the doctor’s discretion.

What happens next

If the doctor needs more detail, you may be asked to:

Book a quick phone/video consult, or
Provide additional information to complete the assessment.
